• Matriculation (Science) with at least 45% marks for diploma courses.
• Intermediate (FSc-Pre Medical) preferred for certain advanced programs.
• Age Limit: 17–30 years.

1. Attested photocopies of Matric and Intermediate certificates.
2. Copy of CNIC or B-Form.
3. Recent passport-size photographs (4 copies).
4. Character certificate from the last attended institution.
5. PMF Registration Fee (as per applicable rules).
